Completed/graded research project
Signed Knowledge Bank License Agreement [PDF]

The instructor must send via Campus Mail or FAX to 292-7859:

Signed Faculty assessment form [Word format]

We will notify you when we have received your submissions, or when some components are missing, but it is your responsibility to insure that all required documents are received before the deadline.
